Be ready for your appointment

All new patients must have current contact information and privacy forms filled out before their visit (download Patient Check-in Forms.pdf).

New patients and patients who have not been seen in three years or more:

Fill out all forms and signatures in the Patient Check-in packet and bring the forms with you to your appointment. If you do not have your completed forms, please arrive 30 minutes prior to your scheduled appointment.

Patients whose contact information or insurance has changed are asked to send us their updated information.

Some insurance plans require a referral from a primary care physician(PCP). Please arrange to have your PCP send a referral to us prior to your visit or bring it with you.

All patients must present a current medical insurance card and picture ID at check-in.

Should a copay be required by your insurance, it will be collected when you check in. We accept cash, checks, credit/debit cards, Apple Pay and Samsung Pay.

All Medicare participants are receiving new cards and ID numbers. When you receive yours, please update our office.

Surgical patients are asked to arrive 15 minutes before appointment time.

If you find it necessary to change or cancel your appointment we request a minimum notice of 2 workdays prior to the appointment. Missed appointments or appointments cancelled in less than 2 work days of scheduled appointment are subject to charges as follows:

  • New patient appointments - $100.00
  • Current patient appointments - $50.00
  • Surgical appointments - $150.00

(Our after-hours answering service cannot schedule or cancel appointments. Please call our office during office hours).
